Objectives

The Diagnostic Clinic asked us to produce an online system to:

Allow the results of a questionnaire to be entered in to the system

To produce a printable report showing graphical measurements of:

Stress

Anxiety

Depression

in addition to explanatory notes.

What we did

We created a web-based solution with security and data protection in mind.

This system generates PDF reports for printing, which can optionally be sent by email, too.

Results are stored in a secured database, which can also be used to store results against clinic patients along with details of any treatments recommended.

A secured login is included to ensure the data is kept safe. Personal data is stored in encrypted form, protecting system integrity.

Results

Our system is used as part of a clinical test for measuring and helping treat the effects of stress, anxiety and depression.

Reports are generated in PDF format and template-based for easy modification of the print layout and content.

Generates a dynamic chart placing arrows on a scale of green (low) to red (high) for stress, anxiety and depression in a format that is easy on the eye.
